Referrals come from Vera House, Dorothy Day House and other emergency shelters for women and women with children. All referrals are taken from the Coordinated Entry List. When in a shelter, a professional with assist them with the referral. If in a place not meant for human habitation they need to contact the Rescue Mission to set up an assessment to be placed on the Coordinated Entry List.
Eligible applicants must:
- Be HUD homeless
- Be at least 16
- Have a documented disability (Supportive Apartments Only)
Residents may have up to two children aged 7 or under (families in the supported apartments can have older children)
Chadwick Residence participants:
- Understand and abide by program requirements
- Work with case manager to develop and work toward goals
- Meet with other staff as required
- Participate in daily activities in the community
- Care for and nurture children
- Respect other program participants
- Do assigned household chores
- Keep personal space neat and clean
- Buy food and prepare meals
- Pay monthly rent on time
- Be in by curfew
- Attend house and other meetings
